Description
Students are responsible for making sure that each entry is approved, evaluated or graded by the teacher who supervised the activity and for completing the necessary formative reflection piece according to graduation portfolio guidelines. Adults are responsible for monitoring student progress and communicating progress to the appropriate people.
Things to Consider
Students should be informed of their responsibilities and given specific guidelines for each entry. A regular schedule must be established for monitoring progress. While an electronic portfolio system makes this process easier, all schools must develop some monitoring process.

What Should Be in Each Portfolio Entry
This checklist, written by the Rhode Island Department of Education, monitors the completion of the entries. This tool is useful for advisors, parents and students to ensure that they are making progress toward the completion of a graduation portfolio. This list should not be used to determine whether the work meets proficiency requirements. Other judgments will be made as to the quality of the work. This checklist is simply to monitor if the entry has the required components.
Year-End Digital Portfolio Review Rubric
This is one example of a monitoring document. Ponaganset High School has developed a yearly monitoring rubric with grade level appropriate criteria. Review your school's specific graduation portfolio requirements and develop your own system for monitoring student progress. Forms like this one simplify the monitoring process and ensure consistency in reporting student progress.
